Please come 15 minutes prior to appointment to fill out paperwork.

1. Insurance ID/ card, other forms of ID

2. Complete list of all your medications and dose

3. Complete medical and surgical history.

4. Any x-rays, radiology studies, and medical records for review

5. Please be prepared for your Co-Pay.

6. A Translator if you do not speak English.
